experienced Oncology Pharmacist for a
Full Time, Day Shift position at a well-respected hospital in Santa Fe.

Duties :
- Clinic pharmacist plays a vital role in improving patient-centered care by supporting the multidisciplinary oncology team, patients, and caregivers.
- They focus on cancer therapy education, toxicity monitoring, and supportive care management.
- They collaborate with oncology providers, contribute to treatment decisions, and ensure adherence to safety protocols. Additionally, they serve as a resource for evidence-based pharmacological treatment options and facilitate access to clinical services.
- Key functions include assessing and selecting appropriate therapies, managing adverse reactions, and providing education to providers and patients.
- Services our clinic pharmacists currently provide (when applicable):
- Prospectively review patient charts to screen for and prevent any medication-related issues
- Evaluate treatment appropriateness based on guidelines and primary literature
- Discuss treatment options based on indication such as initial therapy or relapse/refractory disease
- Evaluate treatment effectiveness based off primary literature
- Provide personalized treatment approaches and optimize drug therapy for the individual patient
- Evaluate safety of medications
Requirements: - NM Pharmacist or ability to obtain
- Oncology Pharmacy experience (~3 years) OR PGYII in oncology
- BCOP (highly preferred)
